Not sure what state you are in, but here, it is the driver's responsibility to ensure that their passengers are buckled up. Other than that, I don't know what to say. I used to be against letting people die if they were "alive". Like when I wanted Terry Schiavo to live. But I have learned since then that there are certain "automatic" brain functions, like being able to track an object across the room with the eyes, that may make you think there is a person in there.

But there's really no person there. Like somebody said earlier, with the brainstem only the automatic functions remain. Whatever it was that made up the person is gone. It's like the soul has left and all that remains is just a breathing shell.

I have nephews that tried to show off and drive fast and I told them to stop and let me drive or else I am calling 911. Whether I like it or not; Hulk Jr. was a minor and John was the adult. So here's the thing.

Sometimes doctors remove part of the skull in order to ease pressure on the brain. There was a girl in Colorado who had this done, and then spent six months or more, going about her life with part of her skull removed because insurance companies didn't want to pay for the operation to put it back. She said she could feel her brain shift, especially when she got up in the morning after laying down all night. When they remove part of the skull from the head, sometimes it is implanted under the skin of the patient's belly, so that the bone has a blood supply and stays alive.

They might have done that to this kid. The angle of the video makes it hard to tell if the head wound is open (unlikely) or if the skin is still intact and covering the brain. In that case it would likely be draping downward into the cavity, rather than being stretched tight across the hole..

In a hospital, having such a large amount of skull removed wouldn't be that big of an issue.

Y'all should read up on Phineas Gage, the guy from the 1800s who was working on a railroad and had a six foot iron tamping rod blown through his frontal lobe and out the top of his head, and survived, albeit with a changed personality.

People with severe epilepsy sometimes have radical surgery to remove large portions of their brain, to stop their seizures. They can often train the remaining portion of the brain to take over for the missing parts. There have been young children with almost half their brain removed, who have gone on to live normal lives.

The brain is freaking awesome. That said, the poor kid is clearly badly damaged and unlikely to be as lucky as Phineas Gage.

He is alive and breathing on his own b/c of the part of the brain that was removed. He had a frontal lobe injury which is the part of the brain located say behind the forehead that is involved with higher cognitive functioning like planning your multi-tasking for the day or suppressing the urge to say something socially inappropriate when you are in a professional setting. The more basic necessary for survival areas of the brain (e.g. automatic breathing) are evolutionarily older and situated more deeply.
Obviously (and the video makes this poignantly clear) that is not to say that frontal lobe injuries aren't very serious. It is a commonly injured area in car/motorcycle/4X4 accidents (your body snaps forward hitting the dashboard say and then snaps back often causing similar damage to the back of the head and other areas of the brain) and bleeeding/bruises in these areas are a HUGE source of long-term disability. The deeper parts of the brain may be what we need to survive, but the frontal lobes and higher structures are crucial to what we think it means to be human- complex thoughts, planning and executing complicated tasks, a lot of personality. I've seen patients, often young men, after 4x4 accidents, car-surfing accidents, etc... who have bruises to these areas (so a mild injury compared to Mr. Graziano) and the family is rightly relieved that their loved one is alive, but as a doctor you have to balance their relief with having honest conversations that it will take time to see if their son truly returns to himself or if there are going to be personality changes and subtle limitations that we make him "not quite the same' for the rest of his life.

I hope that helps some people understand the medical perspective. I wrote this to be accessible to everyone so it is a little over-simplified, but you could definitely go on webmd or emedicine if you wanted more info on frontal lobe injuries.

Who has NEVER gotten behind the wheel after having a couple of drinks? This could have happened to anyone that has. And that young man DID make the choice to not wear his seatbelt. There was a boy in my highschool class who got his arm caught in some kind of tractor part, and it basically slammed him into the ground over and over until a large (and I mean that large) portion of his head was gone. A few months later tho he was back in school just wearing a special helmet to protect his brain.

I in no way, shape, or form condone reckless driving, but this dude should have really been wearing a seatbelt. I'm not saying that anyone deserves anything, but his injury is as much his own fault as Nick's. Sure Hogan's son gets what he deserves given the situation he's brought upon himself -- there's no doubt in my mind about it. Sure it is horrible and I wouldn't be in one of the Graziano's shoes right now. I've seen the uncensored video about his messed up head and did feel a lot for him and for his family, that poor guy probably won't come out of this as his normal self again -- that is if he comes out of this at all.

That said, this story somehow touching me, I watched the entire Hogan interview with Larry King and what I saw was a saddened father overwhelmed by the situation. Are you famous or not, weird or not, sane or not, watching your child suffering in life is NOT a simple thing, whether he/she asked for it or not -- you parents out there, if you love your children, will probably understand what I mean. Is Hogan seeking comfort in religion or anything else of that matter, that's none of our business. Pain is confusing, it is hard, has any of you ever feel it? How did you react? How did you bear with it? We think things, we say things and some of them are things we wouldn't normally think or say. Just remember how hard it is and then imagine yourself having to speak publicly about it, to hear all of them stories, theories and harsh comments coming up every day and somehow having to respond to it in a way that people won't think the least of you. Just imagine the biggest tragedy in your life brought on tabloids by the media, having to deal with both your problems AND the pressure of the world pointing its finger at you? Wouldn't you just lose it? I bet you would. I, for myself, am no robot so I know I would.

All I can say is I feel for those families, BOTH the Hogans and the Grazianos. He is a PVS patient, a persistent vegetative state. He will never get better. And on top of that, he has had a full frontal crantotomy.....so all of the brain cells that allow us to think, hold memories, have emotions, and feel are gone for John. Why is he alive? Because, like all PVS patients, as long as the brainstem and cerebellum are functioning, your heart will continue to beat and your lungs will function. Now he is a trach patient as well, which just compounds the misery. I had to do an entire nursing rotation on these patients, and it was the saddest experience of my life. I wish the families would just let them go, let them die. And the worst part is when the patients open their eyes or make a noise or "smile"....all reflexes, but the family members think otherwise. The whole situation is sad. Its ashame because, unfortunately, stuff like this happens a lot. I dont think age matters. While John may have been 23 (or however old) it doesn't necessarily mean that he should be put at fault for this. Whoever decides to get behind the wheel is responsible. Granted, its not the smartest decision to get behind the wheel with someone who has been drinking. I think that while most people (including myself) are more likely to initially feel for John and not so much for Nick, I think at least some people would be able to feel for both of the guys and the families. Unfortunately the Hogan family is acting like a bunch of uncaring, self centered, fame loving assholes who are lacking any sympathy whatsoever and as if thats not enough they seem completely unable to own up to any of their actions. Which is what makes everyone take the side of John (and rightfully so). Maybe if they were just admit that a mistake was made and no one deserves to live like john is right now, there wouldnt be quite so many people so upset with the entire family.

There are worse things than death. Living like this guy is one of them. He is already pretty much dead. The humane action would be to pull the plug and let his body finally die. The only way this situation would have been just the driver's fault (Nick Hogan) is if this Graziano fella was just walkin down the street minding his own business and drunk Nick Hogan ran him down. Otherwise, I feel a personal responsibility has to come into play here. Like another poster on here said, both of these guys loved to race at dangerous speeds! There is just as much of a chance that things could have gone the other way around and Nick Hogan would be in a coma or dead right now due to this moron. Not that that would be a bad thing! Only at least Nick Hogan could say he was young and stupid! I used to speed when I was like 16 and 17, too. Mr. Gaziano is just plain stupid and immature, and quite frankly got what was coming to him. It's not some gaping wide open hole in his head like everyone's making it out to be. The hole is in his skull. The doctor's removed the damaged parts of the brain and closed the incision. The skin is just sunken over the missing skull.

By a miracle and depending on what part of the brain is removed, I think Graziano could pull through this and regain some normal human functions. Look up ahad israfil. Half his brain was shot out, but he pulled through wayyy beyond anyone's expectations.

I think #188 said it all. A bit harsh but yeah, this poor chap was just in the wrong place at the wrong time. It's not like Nick forced him to get in the car. It's not like he was driving safely himself. Stupid kids do stupid things sometimes as another poster said. Those two were doing stupid things together, had an accident and one of them got badly injured. These things happen all the time.

I have a friend who died because he used to do some crazy things like this. Nobody sued nobody. A careless driver he was and what happened was his fault, not anyone else's fault. Would he have got in a car with a friend who happened to drive like he drove and got injured, well that would have made him irresponsible too.

I think Nick deserves to be punished of course because, well, you do something that stupid you have to pay for it. But that's all there is. It was a fuckin' accident not an evil plan to kill the guy.

The issue here is Hulk made it worse by appearing on Larry King for an hour basically not accepting any responsibility for his son’s accident or admitting that his son was at fault by driving recklessly the night of the accident by racing a Dodge Viper while driving Hulk’s Supra according to witnesses. Hulk is responsible for his son’s actions until he reaches 18. Hulk Jr. already had a prior speeding ticket for going 115 mph in a 70 mph zone. Hulk's attorney was present during the interview and probably told Hulk not to admit any guilt; therefore Hulk should have not done the interview if he really cared and had respect for John and his family. It is obvious all Hulk is concerned about is doing damage control, so he can continue to earn money as a celebrity. I was in shock when Hulk said,” God believes being in coma makes John a better person.”
I can only imagine the hurt, disbelief, and anger when John's family heard Hulk's very self serving and insensitive comment.

First, I'm proud to say I never turned to God when I felt bad. I'm an atheist.
Second, turning to God is one thing. Like praying to God his condition somewhat improves and that his family finds the strenght to go trough this, etc. But saying God wanted Graziano to lose half his skull...so basically saying he totally deserved it and it's a good thing in a way. That's inexcusable and it shows how stupid that man is to make that comment on national TV.
